The purpose of the EczemaLess App Review Program is to evaluate the usability and utility of the EczemaLess app for patients and caregivers to manage Atopic Dermatitis. This review program is being conducted by Polyfins Technology Inc.
Your participation in this research study is voluntary. You may choose not to participate. If you decide to participate in the study and then change your mind, you are free to withdraw at any time with no consequence.
We will do our best to keep your information confidential. All data is stored in a password protected electronic format. To help protect your confidentiality, the surveys will not contain information that will personally identify you. Your identity will not be revealed in any publication or presentation of the results of this research.
Polyfins has the ability to collect your data (including patient-reported data and skin images/photographs collected through the app/software and responses to digital survey questionnaires collected through e- mail/web link). Please note that anonymization of patient skin images/photographs may, despite the application of all available anonymization techniques (including data perturbation) not meet the most stringent requirements of Applicable Data Protection Laws and may be Processed de-identified format only.
The objective of this form is to ask your permission to allow Polyfins, their affiliates, and other third parties at its own discretion to use and share your collected data for multiple data uses across educational and/or business purposes (including marketing/commercial, medical, and digital-related use cases for primary use purposes, as well as the processing of de-identified/pseudonymized data in de- identified/pseudonymized format for secondary use purposes).
